Chapel Enrollment

- We are honored to be asked to pray for your special intentions for others.
  
  Through Chapel Enrollment, you can ask the Sisters of the Presentation to pray for someone experiencing grief, illness, or a life celebration. When you request cards from us, you will receive card(s) to send to the person you want to remember and a form to return to us so we can pray for the person or personal event.
  
  If you would like Sympathy, Get Well, Birthday, or Anniversary cards, please complete the following form. Enter the number of cards you are requesting for each occasion and click on the submit button at the bottom. Alternatively, you may print out the form and mail it to: Sisters of the Presentation, Chapel Enrollments, 281 Masonic Avenue, San Francisco, CA 94118-4416.
